Angels of Colorado, host home program is designed to provide people receiving services under the Home & Community-Based Services (HCBS) waiver the opportunity to live in a home and community of their choosing and to make decisions that best suit their specific needs. Our primary goal is to help individuals create the life they want to live, by providing living options and support to find the best match for the individual. The right match must be person-centered and provide opportunities for meaningful connections. We strive in creating wrap around supports to ensure that individuals with all needs, preferences and abilities get opportunities to live the lives they want.

What is a Host Home?

Host Homes are settings in which a provider opens the door to their home to create an environment where the participant can thrive while receiving all the supports needed.

What is a Host Home Provider (HHP)?

Host Home Provider is an individual or family member who wishes to include an adult with a developmental and/or intellectual disability into their home. A Host Home Provider is a trained and dedicated member of the long-term care team and will assist in every way to ensure that a participant’s goals are achieved.

What is a Family Caregiver?

An individual must be given the option to choose where their services are delivered and have more control over who delivers those services. We help families who seek to provide services to be delivered by family members in the family home or other settings. Angels of Colorado supports family members interested in this model, with inhouse training and 24hour contractor coordination support.
